The disease is genetic in origin, although the inheritance pattern of Fuchs' endothelial dystrophy is unknown.

In some families, Fuchs’ endothelial dystrophy is always inherited in an autosomal dominant pattern (from parents to children). When this condition is caused by a mutation in the COL8A2 gene.

Some people with no family history of the disorder develop new mutations in a gene.
