Hospital at Home (HaH) is a hospital service that offers the patient the alternative of receiving treatment and hospital care at home, under conditions comparable to those provided to conventional hospitalization. HDOM has proven to be a real alternative to conventional hospitalization, being very satisfactory for both the patient and their family environment, as long as it meets quality standards that have been defined by the Catalan Health Service. HaH differs from other home care services in that it meets the criterion of the need for hospital admission or treatment for an acute condition that would otherwise require hospitalization.

The HaH service of the Hospital Clínic de Barcelona depends on the Medical and Nursing Direction and is made up of a multidisciplinary team that provides services to all the Institutes and Healthcare Areas of the Hospital, as well as to the other centres in its area of influence (Barcelona Esquerra, Alliance C-17,…). The HaH works for the comprehensive care of the patient, to help them recover as well as possible with the provision of medical and hospital nurse services in the patient's own home. Its activity has been growing in recent years, being especially relevant during the COVID-19 pandemic. In 2021, with the integration of Plato hospital as a new seat of the Hospital Clínic, the care thar offers has been further expanded. On the other hand, also since 2021, the Home Hospitalization in Mental Health has grown, starting the care for adolescent patients (HaH for Child and Adolescent Mental Health, which depends on the Psychiatry service), with the possibility of attending to up to 11 patients between adults and teenagers. During 2023, the HaH service treated 2,424 patients, with a high degree of satisfaction on the part of users and professionals.

Nowadays, Hospital at Home is an alternative care aimed at patients with both medical and surgical illnesses that require hospital admission. Patients can access the HaH device from the Emergency Department or from the different Hospitalization rooms, and also - in selected cases - they can enter directly from their own home without going through the hospital.
 

What are we dealing with?

The pathologies that are most often treated in the HDOM device at Hospital Clínic are:

- Infectious diseases that require intravenous antimicrobial treatment and/or hospital care, such as complicated urinary tract infections, respiratory infections (pneumonia, COVID-19, flu and others), skin and subcutaneous tissue infections, osteoarticular infections, infectious complications in immunocompromised patients, etc. 

- Acute chronic diseases, such as chronic heart failure, chronic obstructive pulmonary disease, bronchial asthma, ...

- Post-surgical pathology, either after elective surgery, through peri-operative hospital stay reduction programs, or other unscheduled surgical processes (intra-abdominal infections, vascular pathology, complex wounds, ...) 

- Acute psychiatric pathology, both adult and child-adolescent.

Know the team

The Hospital at Home Service is a multidisciplinary team made up of more than 50 professionals: doctors from the Clinical Institutes of Medicine and Dermatology (ICMID), Digestive and Metabolic Diseases (ICMDM) and Neurosciences (ICN), a large team of Nursing, its own administrative staff, physiotherapists, social worker and hospital pharmacist. Coordination and continuity of care are key and we work as a team with the different Institutes, Areas and Centres of the Hospital Clínic, as well as with the other healthcare devices in our area of influence.

Apart from the work of care, the HaH is strongly involved in teaching, research and innovation in different fields of patient care at home.
