The Pharmacy Service

The Pharmacy Service is part of the Medicines Area of the Hospital Clínic de Barcelona. The aim of the Service is to add value to the care process through patient-oriented care, which allows individualized pharmacotherapy with criteria of effectiveness, safety and efficiency. This mission has been developed in collaboration with the healthcare teams responsible for the patient, and within the framework of a territorial pharmaceutical policy or one of strategic alliances.

The Hospital Clínic Pharmacy Service is considered to be  a model service at the state and European level, characterized by an innovative spirit in:

• The application of new technologies and clinical decision support systems in the care and follow-up of patients.
• Maximum safety in the use of the medicine at each stage of the pharmacotherapeutic process.
•  The integration of pharmacists in healthcare teams, who participate in joint decision-making and act as referents and specialists in the safe and efficient use of medicine.
• The application of pharmaceutical care models in the hospital and primary care environment, with a transversal and territorial nature. The establishment of work models, which are the result of collaboration, agreements or alliances between hospitals and pharmacy services, sharing and exchanging knowledge and service provision.
•  The change of the concept of Hospital Pharmacy Service to Territorial Pharmacy Service, not through a Primary Healthcare Center Pharmacy, but rather with a territorial pharmacy team, a unique team, that acts at a territorial level.
• The consolidation of the Service's own lines of research with innovation and impact on an international scale.
