In the event that you need a copy of any document from your clinical history, you can make your request to the Department of Clinical Documentation and Archives by email at documentacioclinica@clinic.cat
Procedure
- If the applicant is the holder of the medical record, he must attach a copy of his identity document (DNI, passport or NIE) to the email.
- If the applicant is a person authorized by the holder, he must attach his identity document, the holder's identity document and an authorization signed by the holder.
