How to obtain  proof of the visit

At the end of the visit you can obtain the proof of the visit from the queue manager. If you have an e-mail, when the visit ends, the proof is automatically sent to the e-mail address on file at the hospital. The patient can request a proof of the clinical act at the time of it, or later if necessary.

How to obtain the proof of the visit for a companion

At the end of the visit, you can obtain the proof of the visit for an accompanying person at the administrative attention points of the service where the care was performed. The accompanying person's receipt will be issued only at the time of the clinical act, and never on subsequent days.

A Proof of visit, test or other type of assistance can be requested as follows:

In person, at the administrative service points of the different services. 

  • By email: ofiregis@clinic.cat
  • By telephone: 93 227 54 00 Extension 2523, from Monday to Friday, from 8 a.m. to 3:30 p.m.
  • HQ Plató: Request in person at the service desk from 8:00 a.m. to 3:30 p.m. or at the Concierge from 8:00 a.m. to 10:00 p.m., and by email at CITA@clinic.cat. Telephone 93 306 99 18 / 93 306 99 29 from 8.00 a.m. to 3.00 p.m.
  • Maternity Center and Villarroel center: Application in person at the service desk during open hours or by email to the contact of the medical service (stated in the notification that the patient receives).

Procedure

If the applicant is the holder of the medical record, together with the application, a copy of the applicant's identity document (DNI, passport or NIE) must be attached. The request must indicate the period of time that must be confirmed.

If the applicant is a person authorized by the holder, together with the application, the person must attach his or her own identity document, the identity document of the holder and an authorization signed by the holder. The request must indicate the period of time that must be confirmed.
